FINAL REPORT

on

Investigation of Aircraft Accident with An-12 aircraft, registered LZ-BRP, owned by Air Operator Bright Aviation Services, occurred on 01.01.2004 at Tehran Airport, Islamic Republic of Iran.

On the 01.01.2004 at 16:55 UTC during the landing of An-12 aircraft of Bulgarian air operator on runway 29R at the Teheran Airport at touch-down the aircraft commander, piloting pilot, set the throttle of engines No1 & No2 on the left-hand half-wing at ground idle position. As a result of the emerged yawing moment the aircraft turned to the left and despite of the consequent activities of the crew it left RWY 29R, crossed the ground between the RWY 29L and RWY 29R, hit a concrete column 1.6 m high with the left-hand fore part of the fuselage and tore the skin in the area between 6th and 19th strength ribs. The aircraft continued on runway 29L and stopped, whereupon on self-moving and accompanied by a pilot car and fire-engine, taxied to the defined tarmac. There were no consequences for the crew. Consequences for the aircraft: crippling of the skin between the 6th and 19th strength ribs in the area of the pressurized and cargo compartments. In accordance with the additional provisions of Regulation No 13/27.01.1999 of the Ministry of the Transport, the occurrence was classified as an Air Accident.

investigation of a serious incident with Ка-26 helicopter, reg. LZ-6013, owned by Balkan Agro Aviation Ltd Air Operator, town of Vratsa, occurred on 02.04.2004.

On the 02.04.2004 the commander of Ka-26 helicopter, reg. No LZ-6013, was performing his fifth ACW flight for the day.

After the engines start and check the pilot took-off from a temporary landing site in Chomakovtsi village area. He treated the first block planned and flew for treatment of the second one, where he performed two working passes. At the end of the third pass an intense vibration of right-hand engine started. The commander climbed up to 50 m height, when the engine self-shutting down occurred. The pilot saw a smoke from the right-hand engine in the retrospective mirror. He chose an appropriate site and started left-hand turn for a forced landing. The helicopter commander performed forced landing in straight ahead direction in heading 190° on a lawn, situated at 750 m southwest of Chomakovtsi.

investigation of an air accident with a motor hang glider delta "Lebed I", owned by Avio-Delta Ltd, town of Kazanlak, occurred on 20.06.2004.

On 20.06.2004 the pilot fulfilled the first flight for the day. After start and check of the engine the pilot started a take-off for repositioning in the landing ground area. During the flight at extremely low altitude over the ground the motor hang glider hit the ground and the wing was totally destroyed. The pilot was seriously injured and was hospitalized in Kazanlak Hospital. In accordance with Para.3 of Additional Provisions of Regulation 13 of the Ministry of Transport of 27.01.1999 about aircraft accident investigation the aviation occurrence has been classified as an air accident.

investigation of an aviation occurrence with P92 Echo-S airplane, reg. No LZ-BAT, owned by Cabelcommerce Ltd, Sofia, occurred on 02.07.2004 in the area of Dolna Banya Airfield.

On the 02.07.2004 the commander of P92 Echo-S aircraft reg. No LZ-BAT fulfilled the second flight for the day. After engine start the commander taxied and took-off from runway 09 of Dolna Banya Airfield for a training flight over the runway. After the take-off the aircraft climbed and made a turnover for a touch-and-go on RWY27. After the touch-and-go the aircraft according to the witnesses’ information the aircraft flew about 200…300m away from the threshold of the runway in west direction, started a steep right-hand turn in climb, climbed about 60…80m, then started left-hand turn with steep bank to approach to RWY09. During the turn the aircraft lost altitude and in a course of 120…130° it hit the ground.

The pilot received injuries with fatal outcome and the aircraft was fully destroyed
